New online health pathway creates ‘virtual health community’

The Darling Downs Hospital and Health Service has linked up with the Darling Downs West Moreton PHN to launch a new web portal to ensure patients are receiving the most appropriate care, in the right place and in the right time.

The Darling Downs HealthPathways Program is a new web portal that details local health care providers.

The portal is accessible to public and private health clinicians looking to refer patients in their care to more specialised treatment.

The information ensures health clinicians, including GPs and allied health professionals, have the most up-to-date referral pathways.

"The program creates a virtual health community," Darling Downs HealthPathways clinical director and Stanthorpe Hospital Medical Superintendent Dr Dan Halliday said.

"It increases our public and private hospitals’ and community services’ ability to provide the right type of care, within clinically recommended timeframes, to all patients.

"It also aims to improve a patient’s journey through the health system to ensure they have consistent and equitable access to public medical and surgical services, if required."

Darling Downs and West Moreton PHN Chief Executive Officer Merrilyn Strohfeldt said the web portal would support GPs and clinicians with assessment, management and local referral information, including important referral pathways for mental health and drug and alcohol treatment services.

"As many GPs and clinicians know, the health environment is continually changing and at times can be difficult to navigate," Ms Strohfeldt said.

"HealthPathways ensures that we are able to reduce fragmentation by making the patient journey more seamless and move towards a more ‘connected’ system that improves patient outcomes.

"HealthPathways has been specifically developed by GPs for GPs to ensure that patients in the region receive the right care, at the right time, and in the right place."

The program has also included the implementation of a formal Clinical Prioritisation Criteria (CPC) across the Darling Downs Hospital and Health Service area.

The CPC is a state-wide system that helps ensure patients are referred to public specialist outpatient services in order of clinical urgency.

The Darling Downs HealthPathways Program will be launched officially on 14 June.
