Discharge Planning will commence from the time you are admitted to hospital in consultation with you and your carer/s.
Discharge time is usually 10am (this time may vary). Please arrange in advance for your family/friends to assist with your transport home from hospital where possible. If this is not possible, please advise nursing staff to facilitate alternative arrangements for your discharge.
Should you be unable to leave the hospital at 10am on day of discharge, it may be arranged for you to await discharge plans in the Admission, Discharge and Transfer Lounge where you will be continued to be cared for until discharge time.
You will be provided any scripts that you require for an ongoing treatment. Scripts can be filled at the Toowoomba Hospital Pharmacy or your local Pharmacy. Normal charges apply.
Follow up appointments
Follow up appointments will be discussed with clinical staff prior to discharge, or sent to you in the mail.

Discharge Summary to your Doctor
A Discharge Summary of your hospital admission will be sent to your Doctor after your discharge.
As a busy regional hospital, we have many people who require care. Should you need to remain a little longer in hospital to recover and no longer require specialist care, you may be transferred to another hospital within the Darling Downs.
