TGA approved vaccinations

Your vaccination will be recognised if it's either:

  • TGA approved and you were vaccinated in Australia
  • WHO-COVAX endorsed if you were vaccinated overseas.

Not all WHO-COVAX endorsed vaccines are TGA approved for use in Australia. If your vaccine isn't linked to the Australian Immunisation Register, you'll need to provide evidence it's WHO-COVAX endorsed.

COVID-19 vaccination evidence

There are a few ways you can show us that you've been vaccinated against COVID-19.

Along with the documents listed below, you'll need to fill in a COVID-19 evidence form [PDF 236.6 KB].

Make sure your vaccine is on the Approved and recognised COVID-19 vaccination list [PDF 587.03 KB].

If you have a Medicare card

If you have a Medicare card, you can download your COVID-19 digital certificate from your Medicare account via MyGov.

You can also use your immunisation history statement as evidence.

You can get this by calling the Australian Immunisation Register on 1800 653 809 or by asking your regular healthcare provider.

If you don't have a Medicare card

If you don't have a Medicare card, you can get proof of your COVID-19 vaccinations by:

You can also give us a letter from a medical practitioner, vaccine service provider or other health professional.

The letter must be on letterhead, signed and include their professional designation, service provider number (if applicable) and practice stamp.

Vaccine preventable diseases evidence

We have information below about the types of evidence we accept for vaccine preventable diseases.

You can talk to your hiring manager or placement coordinator if you're:

  • not sure how to provide your evidence
  • not able to meet the vaccination requirements for some reason.

If you're a student, you should should talk to your education provider.

Documenting your evidence

Use the following forms to document your evidence. You may only need to use one, depending on the requirements and documents you have.

VPD evidence certification form [PDF 83 KB] - ask your medical practitioner, registered nurse, or occupational health provider to fill in this form on your behalf.

VPD evidence form [PDF 103 KB] - use this form if you already have copies of acceptable forms of evidence. You must attach copies of the documents listed in the table below.
