Clinical Excellence Division

The Statewide strategy for end-of-life care 2015 is intended to integrate care at the end of life as a core element of health services in Queensland.

The Clinical Excellence Division coordinates the implementation activities arising from this strategy in collaboration with Hospital and Health Services (HHSs).

Major activities

  • Develop, promote and support HHS's to implement a statewide model for Advanced Care Planning (ACP) including standardised documentation and patient resources
  • Ensure documentation regarding care at the end of life can be included within Queensland Health’s integrated electronic medical record
  • Evaluate the effectiveness of statewide education programs for care at the end of life and re-design as necessary in line with the Statewide strategy for end-of- life care 2015
  • Collate, or develop where necessary, a standardised set of health professional educational materials to be hosted on an intranet hub
  • Develop a standardised dataset and key performance indicators to evaluate and benchmark care provided at the end of life
  • Collate, or develop where necessary, a standardised set of evidence-based patient assessment and care management tools for implementation by HHS
