Applications for approval to acquire

Under section 23 of the Radiation Safety Act 1999, a person must not acquire a radiation source, unless the person is a possession licensee for the source and the holder of an approval to acquire the source.

Application for approval to relocate

Under section 25 of the Radiation Safety Act 1999, a person must not relocate a radiation source, unless the person is a possession licensee for the source and the holder of an approval to relocate the source. In this context, ‘relocate’ means to permanently send a radiation source to another jurisdiction (ie. outside Queensland).

Application for approval to dispose

Radioactive substances

Under section 26 of the Radiation Safety Act 1999, a person must not dispose of radioactive material unless the concentration or activity of a radionuclide in the material is not more than the maximum concentration or activity prescribed under part 4 of the Radiation Safety Regulation 1999. If material is to be disposed of which is greater then the prescribed concentration or activity, then approval for such disposal is required.

Radiation apparatus (x-ray equipment and class 4 lasers)

Under section 27 of the Radiation Safety Act 1999, a person who disposes of a radiation apparatus must advise Queensland Health, in writing, within 7 days of the disposal. There is no application form relating to the disposal of radiation apparatus.

Application for approval to change a radiation safety and protection plan

Under section 31 of the Radiation Safety Act 1999, a possession licensee may apply to Queensland Health to change the licensee’s approved radiation safety and protection plan for a radiation practice.

An application must be made in the approved form.

Application for the issue of another Act instrument (licence, certificate, approval)

Under section 101(1) of the Radiation Safety Act 1999, the holder of an Act Instrument may apply to Queensland Health for the replacement of the instrument if it has been lost, stolen, destroyed or damaged.

An application must be made in the approved form.

Application for approval as a Medical Physicist

The following  application form should be used to be approved as a medical physicist for the purposes of the ARPANSA "Code of practice for the exposure of humans to ionizing radiation for research purposes (2005)."
