This page provides advice on how to apply for licences to possess and use ionising radiation sources (ie. x-ray equipment) for diagnostic radiography, radiation therapy or nuclear medicine.

Medical radiation practitioner - use licence application kit
Any person who uses radiation source for diagnostic or therapeutic purposes must hold a use licence issued under the Radiation Safety Act 1999. To apply for a licence, the following steps should be followed:
Read the following information sheet which outlines the process by which medical radiation practitioners may apply for a licence to use radiation sources. It also outlines the process student medical radiation practitioners must follow to obtain a licence to use radiation apparatus as part of their clinical training.

Download and complete one of the following application forms.
Note: Form 7 is intended to be used by the majority of medical radiation practitioners seeking licences to use radiation sources. This application is intended for electronic completion to facilitate fast-tracking of applications.
If Form 7 cannot be completed using the selections available within the form, Form 2 must be used.

Submit the completed application form, with the supporting information as outlined in the application form, to Radiation Health. The prescribed fee must accompany the application.
Possession licence application kit
Any person who possesses a radiation source for a diagnostic or therapeutic purpose must hold a possession licence, issued under the Radiation Safety Act 1999. To apply for a licence, the following steps should be followed:

Develop a radiation safety and protection plan which details the radiation protection measures that will be conducted by your practice. Information to assist in the development of such a plan is available, as follows:
Submit the completed application form, with a signed and dated radiation safety and protection plan, to Radiation Health. The prescribed fee must accompany the application.
