Rehabilitation
About the service
The SCHHS Rehabilitation Service provides intensive interdisciplinary rehabilitation programs to patients with reduced function and independence due to an acquired illness or injury. Rehabilitation programs are based on individualised goals to optimise a person’s ability to safely return and/or remain in the community with additional services, supports and adaptive equipment as required.
Conditions managed within rehabilitation include, but are not limited to:
- stroke
- acquired brain injury
- neurological conditions (e.g. Multiple Sclerosis, GBS)
- amputation
- spinal cord injury
- multi-trauma
- various orthopaedic conditions (e.g. post-operative knee or hip replacements), and
- deconditioning following medical or surgical illness.
Long term complications associated with the above conditions may also be managed within the Rehabilitation service eg. Spasticity management in UMN conditions such as stroke or MS.
Patients requiring an intensive therapy block may be admitted to one of the three inpatient rehabilitation units and those that can achieve their goals through ambulatory rehabilitation services will be referred on to the appropriate community based program following assessment in one of our specialist rehabilitation outpatient clinics.
Inpatient services are offered at:
