How to apply for information

Medical records
Applications made by individuals and agents may be lodged in person (over the counter) at the health service facility where treatment was provided.
Queensland Police Service personnel
Routine requests should be lodged by email TCHHS-RTI-Privacy@health.qld.gov.au, and promptly forwarded to the Director, Nursing and Midwifery of the facility.

Certified documents to support an application (e.g. a marriage certificate as evidence of relationship) CANNOT be accepted as scanned attachments to an email.
- The application and certified supporting documents can be lodged by post, or
- The application lodged via email indicating that the certified supporting documents have been posted.

In most cases a valid application will include the email as a cover with documents in pdf (portable document format) as attachments, such as:
- The application (signed)
- A letter printed on the firm's letterhead and signed
