Resource Directory - Hospital in the Nursing Home (HiNH) Program

Overview

The HiNH program provides a safe and appropriate alternative to hospitalisation. The program reduces the need for residents to leave Residential Aged Care Facilities RACFs) to attend or be admitted to an acute hospital for conditions that may be safely treated within their own facilities.

A Clinical Nurse Consultant will coordinate the episode of care in partnership and consultation with the resident's own General Practitioner, Aged Care Facility staff and the Hospital staff. This will allow the resident to remain in their own facility for treatment where possible.

The Program Objectives

  • Provide substitution for hospitalisation through avoidance or early discharge,
  • Reduce presentation and admissions of residents from RACFs to ED,
  • Improve continuity of care for residents of RACFs.

Services provided

  • Outreach and consultation service,
  • Clinical support and education for residents and family,
  • Regular comprehensive education programs for aged care facility staff,
  • Resource manual to assist staff,
  • Contact details for medical and expert advice for staff.

Eligibility for this Service

  • Referral by ED,GP,RACFand Hospital staff,
  • Informed consent.

Exclusion Criteria

  • Clients who require inpatient care not covered by the Program management guidelines,
  • Resident, carer, family,GP,RACFreluctance to participate in the program,
  • Residential clients outside of TheTownsville Health Service District.
