Patients with a diagnosis of stroke, spinal cord injury, amputees, brain injury, neurological disorder and reconditioning after trauma or major surgery/prolonged illness who are able and willing to participate in an intensive multidisciplinary program are able to be referred. Access to our service is via a General Practitioner/Treating Doctor referral.

Once the referral has been received an appointment or review of the patient will be completed at the earliest convenience.
