Sub Acute Chronic Care Rehabilitation (SACCR)
Our team of health professionals, students and supporting staff provide treatment, education and tools to help you manage chronic disease at home. We aim to reduce the need for future hospital admissions. We communicate with your doctor so they are kept informed of your progress.
The clinic is a working partnership with CQUniversity.
