Hospital in the home
The Hospital in the Home service provides you with high-level acute care in the comfort of your own home so you don’t have to stay in hospital. The service is organised for an agreed period of time to help you during your recovery. For the duration of your care you will remain a patient of the hospital.
Services
Hospital in the Home provides medical, nursing, occupational therapy, pharmacy and social work services.
Services are provided at your home, in the HITH clinic, and via Telehealth.
Conditions commonly managed in the home include:
- skin inflammations
- blood clot/s in the legs or lungs
- pneumonia and severe chest infections
- urinary tract infections
Who can access this service
You are eligible for this service if you:
- are a public hospital patient who without HITH would remain in hospital
- are medically and mentally stable
- are within the CQ Health service area (50km from Rockhampton Hospital)
- need a minimum of daily treatment or assessment
- have a working phone and a fixed address
- have a safe environment (for you and the staff who visit you)
- consent to the program
How to access this service
You can be referred by your GP or a hospital doctor, nurse practitioner or allied health professional.
Ask your doctor whether you can receive care at home. To decide whether the Hospital in the Home service is safe and suitable for you, your doctor will consider your individual care needs, health condition and home environment.
