This program aims to maintain frail elderly persons living at home by accessing a range of services or to enter a residential care facility when appropriate. This service also assesses and applies for assistive aids to help eligible clients at home.

This multi disciplinary team includes Allied Health assessment and therapy and Child Health Nurses who provide care,  assessment and education. Well Babies Health Clinics are held at the Bolsover Street Health Clinic, Phillips Street Health Centre North Rockhampton, Gracemere, Mt Morgan and other outreach centres. Services include:

Our objective is to increase injecting drug user (IDU) access to sterile injecting equipment. We also provide confidential access to education and resources that reduce the incidents of injecting related injury and disease and blood borne virus amongst IDU. We facilitate and promote the safe disposal of used injecting equipment and improve IDU access and referral to drug treatment programs, healthcare and other services. The program plays a key role in supporting community based health promotion programs based on a Harm Minimisation approach.

CANAS provide a safe alternative to acute hospital care and supports early discharge. Nursing and Allied Health staff deliver treatment at home, in clinics or in a residential care facility. This includes medication (oral and IV), wound  management, equipment and education. CANAS link clients back to community services after discharge if required.

CANAS empowers clients to make decisions about their health and lifestyle and provides the opportunity to make these choices with informed support from caring health professionals.
