Acute Geriatric Evaluation Service (AGES)
Providing Clinical Support to Residential Aged Care Facilities
The Acute Geriatric Evaluation Service are a team of health professionals specifically specialising in AGED Care, the AGES team consists of
- Geriatrician
- Registrar Advanced Trainee
- Nurse Practitioner
- Nurse Navigators
The Acute Geriatric Evaluation Service focuses on five main components:
- Telephone triage service: providing advice and triaging of RACF residents before a hospital admission is required (in concert with the GP)
- Working with GPs: promoting cross sector exchange of information, as well as education about the AGES–Nurse Navigator model of care
- Tele-Health consultations: to provide virtual assessment and reviews e.g. via portable IT device
- Rapid Assessment Team (Mon-Friday 0800-1800 hours): Mobile assessment, care and treatment in the RACF by the AGES team
- Advocacy, education, training and support to RACF staff
