Access under the Right to Information Act 2009
Make a request under the Right to Information Act 2009 when you are seeking access to documents that:
- Are non-personal in nature, for example, documents about a new government initiative
- Contain both personal information relating to yourself or on behalf of another person and may contain the personal information of another person and/or non-personal information, for example documents about a recruitment process for a Queensland government job you applied for, including job applications submitted by other people
- Are the medical records of a deceased person, including by a family member.

What is required before the application can be processed?
- Be on the approved form, see above "how to apply"
- Payment of the prescribed application fee
- Evidence of identity and authority as specified on the application form
- Include a postal address or an email address for us to reply to
- Be as clear as possible about the documents you are seeking.
Agents proof of authority – other information (non-personal)
It is good practice to provide your authorisation when making any application on behalf of an applicant (a person or organisation). Without evidence of authority we have no surety that the information is actually being released to the applicant.
Access and amendment under the Information Privacy Act 2009
Make a request under the Information Privacy Act 2009 when you are seeking access to:
- Personal information relating to yourself or on behalf of another person, for example, documents about your medical treatment in a public hospital, or
- When you are seeking amendment to your personal information that is incorrect or which you do not agree with. You will need to provide details of why the information may be inaccurate, incomplete, and out of date or misleading.
