Care Plan for the Dying Person Digital Uplift Project
The Care Plan for the Dying Person Digital Uplift Project is a statewide quality improvement initiative that aims to align end-of-life care with best practice in the integrated electronic Medical Record (ieMR).
New and improved care plans, fact sheets, quick reference guides and an education package are now available as part of the Care Plan for the Dying Person Digital Uplift Project.

Family and carer information pack
Clinical services can use the family and carer information pack to provide families and carers with information about what to expect when a loved one is dying.
The pack includes the family and carer information fact sheet (PDF 256 kB) for distribution to families and carers.

Using palliative care outcome tools in ieMR
Palliative Care Outcomes Collaboration
Palliative Care Outcomes Collaboration (PCOC) is a national program that supports the routine assessment and measurement of outcomes for people receiving palliative care.
Funded by the Australian Government, PCOC uses standardised clinical tools to help palliative care services assess patient needs, monitor changes over time, and inform care planning and clinical decision-making.
PCOC Version 4 now available in ieMR
Version 4 includes updates to the five core clinical tools, including ESAS-PCOC, an expanded symptom assessment system, and new delirium measures to support assessment of delirium and related distress.

About the unit
The Statewide Specialist Palliative Care Digital Health Support Unit works with specialist and generalist palliative care services across Queensland by:
- developing standardised digital health resources
- contributing to dashboard development and maintenance
- providing mentoring in digital health.
This work helps identify areas for improvement, enables data-driven decision-making, and promotes best practice in digital health to improve palliative care delivery.
