Community Based Palliative Care Education Project

Project Overview

The Centre for Palliative Care Research and Education (CPCRE) was engaged by Brisbane North PHN to develop, deliver and evaluate palliative care education and support pathways for after-hours general practitioners and ambulance paramedics, from 1st January 2016-30th June 2016.

Project Information

Funded by the Australian Government Department of Health, through Brisbane North PHN (BNPHN), the project provided palliative care education to after-hours GPs and ambulance paramedics in the Brisbane North region. Project Owner was Helen Hoare, Manager Primary & Community Care Coordination with BNPHN.

Project officer Leanne Clemesha, responsible for GP education, has a background in clinical palliative care as well as within Queensland Health.

Senior Clinical Educator with Queensland Ambulance Service (QAS), Chris Hodge, developed education materials for QAS paramedics, with an initial emphasis on Brisbane North. QAS plans to eventually roll out the palliative care education across Queensland.

CPCRE Director of Learning and Development John Haberecht was Project Manager. An Expert Working Group included palliative care physicians from the University of Queensland, palliative care services at RBWH, The Prince Charles Hospital, Redcliffe Hospital, St Vincent's Hospital, and Children's Health Queensland's Paediatric Palliative Care team, as well as representatives from Queensland Ambulance Service and general practice. That group advised on the content of the education material, primarily for the after-hours GPs.

The Project Steering Committee, chaired by Helen Hoare, PHN Manager, Primary & Community Care Coordination, included consumer representation, as well as from MNHHS, QAS, GPs, and Medical Deputising Services.

The team established links with other initiatives such as the MNHHS review of palliative care, the statewide rollout of the End of Life Strategy, and the Map of Medicine tool through the PHN. It also established links with organisations such as:

  • Australian Medical Association Queensland
  • Royal Australian College of General Practitioners
  • Australian College of Rural and Remote Medicine
  • CheckUP
