Metro North HHS Advance Care Planning 

MNHHS offers an ACP service with support from dedicated ACP Facilitators based at each hospital and in the community. The Facilitators support clinical staff through training and education programmes to increase awareness and knowledge of the range of ACP documents and confidence with introducing the topic of ACP.

The Facilitators can provide training and education tailored to the needs of the clinician or clinical area. This may include focussed training with identified ‘ACP Champions’.  This service is available to all care providers in the Brisbane North area inclusive of private RACFs.

Facilitators are also available to assist with undertaking in-depth conversations with complex clients and the completion of ACP documents.
