Collection notice: The Department of Health is collecting your information in order to process your report about alleged breaches of the Tobacco and Other Smoking Products Act 1998. Personal information collected by the Department of Health is handled in accordance with the Information Privacy Act 2009. The personal information provided by you may be provided to Public Health Units within Hospital and Health Services for the purpose of investigating the alleged breaches of the Tobacco and Other Smoking Products Act 1998.
Your personal information will not be disclosed to other third parties without consent, unless the disclosure is authorised or required by under law. If the personal information is not collected, the Department of Health may not be able to properly investigate your reported breach.
