ACP activity and monitoring

Resources and tools are available to support health services to:

• easily monitor and measure their advance care planning (ACP) activity
• implement related quality improvement activities
• demonstrate compliance with Queensland and national ACP-related health care standards, specified in the National Safety and Quality Health Service (NSQHS) Standards (Action 5.09 and 5.17) and the strengthened Aged Care Quality Standards (Outcome 3.1.6).

Queensland Hospital and Health Services can access information about available ACP activity reports on the Queensland Health Intranet (QHEPS) Advance care planning hub.

Queensland Primary Health Networks can request access to ACP activity reports for their regions via the Statewide Office of Advance Care Planning.
