The Statewide Office of Advance Care Planning supports patients, families, carers and health professionals with information and resources about advance care planning.
Advance care planning (ACP) involves patients thinking about and making decisions now to guide their future health care, for a time when they cannot make decisions for themselves.
The Queensland Health advance care planning clinical guidelines (PDF 2868 kB) are now available to support health facilities with the implementation of the advance care planning process.
