Forms for Non-Queensland Health Facilities

Queensland Health has advised that non-Queensland Health facilities may use Queensland Health (QH) forms such as the Subcutaneous Medication Infusion Device Chart.

Where forms such as the Subcutaneous Medication Infusion Device Chart are used by non-Queensland Health facilities, QH must be appropriately acknowledged on the form. No change may be made to forms because their safety features are rigorously tested, and any changes may affect those safety features.

Queensland Health's Medicines Regulation and Quality will provide the latest version of the form for printing, a user guide, and also organise copyright and Intellectual Property provisions.
