Browser Error Messages

Getting an error message ‘The requested URL was rejected. Please consult with your administrator.’

The MASS-eApply site certificate has been updated and an old version is cached in your browser.

Troubleshooting:

  1. Clear cookies and cache for https://forms.health.qld.gov.au/ in your browser settings.
  2. Reload the webpage.

Google Chrome (Desktop)

  1. Go to https://forms.health.qld.gov.au/
  2. Click the Tune icon (two sliders) or the lock icon located at the left side of the URL bar.
  3. Select Cookies and site data or Site settings.
  4. Click Manage on-device site data or Delete cookies and site data.
  5. Click the Trash can (Delete) icon next to the listed data, then select Done.
  6. Reload the page to apply changes.

Alternative method:

  1. Click the three-dot menu in the top right and select Settings.
  2. Select "Privacy and security" from the left menu.
  3. Click on "Site settings".
  4. Click "View permissions and data stored across sites".
  5. Use the search bar in the top right to find https://forms.health.qld.gov.au/
  6. Click the trash can icon next to the site name or click the site name and choose "Delete".

Google Chrome (Android/iOS)

  1. Go to https://forms.health.qld.gov.au/ in the Chrome app.
  2. Tap the tune icon (two sliders) or lock/info icon to the left of the address bar.
  3. Tap Cookies and site data.
  4. Tap the trash can icon or Delete

Safari (Desktop)

  1. Select Safari in the top menu bar > Settings (or Preferences).
  2. Go to the Privacy tab.
  3. Click Manage Website Data.
  4. Search for https://forms.health.qld.gov.au/, select it, and click Remove.

Safari (iOS)

  1. Open Settings > Safari.
  2. Tap Advanced > Website Data.
  3. Search for https://forms.health.qld.gov.au/ and swipe left to remove.

Microsoft Edge

  1. Click the ... menu (top-right) and select Settings.
  2. Navigate to Privacy, search, and services.
  3. Under the Clear browsing data section, click Choose what to clear.
  4. Alternatively (more precise): Under Cookies and data stored, click Manage and delete cookies and site data > See all cookies and site data.
  5. Search for https://forms.health.qld.gov.au/ in the search box.
  6. Click the down arrow and select Delete to remove that site's data.

Firefox

  1. Click the ... menu (top-right) and select Settings.
  2. Navigate to Privacy, search, and services.
  3. Under the Clear browsing data section, click Choose what to clear.

Alternative method:

  1. Under Cookies and data stored, click Manage and delete cookies and site data > See all cookies and site data.
  2. Search for https://forms.health.qld.gov.au/ in the search box.
  3. Click the down arrow and select Delete to remove that site's data.
