Browser Error Messages
Getting an error message ‘The requested URL was rejected. Please consult with your administrator.’
The MASS-eApply site certificate has been updated and an old version is cached in your browser.
Troubleshooting:
- Clear cookies and cache for https://forms.health.qld.gov.au/ in your browser settings.
- Reload the webpage.
Google Chrome (Desktop)
- Go to https://forms.health.qld.gov.au/
- Click the Tune icon (two sliders) or the lock icon located at the left side of the URL bar.
- Select Cookies and site data or Site settings.
- Click Manage on-device site data or Delete cookies and site data.
- Click the Trash can (Delete) icon next to the listed data, then select Done.
- Reload the page to apply changes.
Alternative method:
- Click the three-dot menu in the top right and select Settings.
- Select "Privacy and security" from the left menu.
- Click on "Site settings".
- Click "View permissions and data stored across sites".
- Use the search bar in the top right to find https://forms.health.qld.gov.au/
- Click the trash can icon next to the site name or click the site name and choose "Delete".
Google Chrome (Android/iOS)
- Go to https://forms.health.qld.gov.au/ in the Chrome app.
- Tap the tune icon (two sliders) or lock/info icon to the left of the address bar.
- Tap Cookies and site data.
- Tap the trash can icon or Delete
Safari (Desktop)
- Select Safari in the top menu bar > Settings (or Preferences).
- Go to the Privacy tab.
- Click Manage Website Data.
- Search for https://forms.health.qld.gov.au/, select it, and click Remove.
Safari (iOS)
- Open Settings > Safari.
- Tap Advanced > Website Data.
- Search for https://forms.health.qld.gov.au/ and swipe left to remove.
Microsoft Edge
- Click the ... menu (top-right) and select Settings.
- Navigate to Privacy, search, and services.
- Under the Clear browsing data section, click Choose what to clear.
- Alternatively (more precise): Under Cookies and data stored, click Manage and delete cookies and site data > See all cookies and site data.
- Search for https://forms.health.qld.gov.au/ in the search box.
- Click the down arrow and select Delete to remove that site's data.
Firefox
- Click the ... menu (top-right) and select Settings.
- Navigate to Privacy, search, and services.
- Under the Clear browsing data section, click Choose what to clear.
