Submitting quarterly reports
From 1 February 2017, public hospitals with inpatients, public residential aged care facilities, and private health facilities licensed under the Private Health Facilities Act 1999 are required to submit quarterly reports, summarising the results of tests taken for Legionella in accordance with the facility’s water risk management plan.
The person in charge of the facility must submit the report by:
- completing the Legionella periodic report form (PDF 119 kB)
- emailing the form to email@example.com.
Private residential aged care facilities are not yet required to submit quarterly reports to the Department of Health.
The person in charge of the facility must submit the report within 30 business days from the end of the reporting period. Please see the table below for respective future submission dates.
