CCAQ clinical coding query form

Use this form to submit coding queries relating to the International Statistical Classification of Diseases and Related Health Problems, 10th Revision, Australian Modification (ICD-10-AM) / Australian Classification of Health Interventions (ACHI) / Australian Coding Standards (ACS) to Clinical Coding Authority of Queensland (CCAQ).

  1. Define your coding query.
  2. Research existing resolutions to your query by researching IHACPA’s ICD-10-AM/ACHI/ACS, National Coding Advice, coding advice for similar queries from Clinical Coding Authority of Queensland (CCAQ) below and other Australian jurisdictional coding authorities, and have your query reviewed by another experienced coder.
  3. If your coding classification query is not resolved and further clarification is required, submit your query to CCAQ using this form.
  4. CCAQ will review your query and respond with advice.
  5. CCAQ may forward queries to the ACE for further advice.

Note:

  • Queensland coders can refer to coding advice from other Australian jurisdictional coding authorities for a similar coding query if relevant advice and guidance is not available through IHACPA, CCAQ, or QHAPDC. However, if there is conflicting advice between jurisdictions, then a coding query needs to be submitted to CCAQ.
  • Coding query responses relevant statewide are published to the Clinical coding queries | Queensland Health.
  • Remove all identifiable information relating to any patients and clinical staff.
  • Your coding query may be summarised to be more succinct. We do not publish your details or your facility details.
