Proactive strategies
The following strategies are part of our preventative health measures.
Primary prevention
Comprehensive health advice and support to promote overall wellbeing and to prevent diabetes.
Primary health prevention includes:
- public health policy changes
- community health campaigns and education
- funding incentives
- vaccination programs
- improving access to health care services.
Secondary prevention
Targeted recommendations for people at risk of diabetes due to:
- hereditary factors
- nutrition and physical activity behaviours
- cultural influences
- age and being overweight or obese.
Secondary health prevention includes the use of screening tools to identify people at risk of developing a chronic illness and providing early intervention with appropriate support services.
Tertiary prevention
Risk management and slowing the progression of the chronic effects associated with diabetes, such as managing blood sugar levels and preventing complications.
Tertiary health prevention supports people with a chronic condition with interventions to minimise the long term effects of their condition.
Programs and services
The Commonwealth Health Department fund health assessments for people between 45 and 49 with a higher risk of developing a chronic illness.
They also fund specific health assessments for First Nation peoples for early detection of chronic conditions from 15.

- My health for life is a free health coaching chronic condition prevention program that supports adults at risk of developing conditions such as type 2 diabetes, heart disease and stroke.
- Self Management of Chronic Conditions - SMoCC is a chronic disease program that offers free phone support for people with conditions such as pre-diabetes and type 2 diabetes.
- MindSpot - A free digital psychology service supporting people with chronic health conditions and chronic pain.
- Health and Wellbeing Queensland has a range of health and wellbeing prevention programs aimed at improving the health and wellbeing of Queenslanders.
- Way to Wellness - digital health check with phone coaching for patients to optimise their health and wellbeing while on Queensland Health waiting lists.
