HealthPathways
Many clinicians will first encounter Clinical Prioritisation Criteria (CPC) when making a referral via HealthPathways.
HealthPathways is a secure, web-based clinical guidance portal that provides evidence-informed information on the assessment, management and referral of common clinical conditions. It is designed to support clinicians — particularly general practice — to make the right decision at the right time, and to access clear, consistent referral guidance.
Pathways are developed by General Practitioners in partnership with local hospital specialists, allied health professionals and other subject matter experts. This collaborative approach ensures content reflects both best available evidence and local service availability, referral processes and models of care.
Across Queensland, HealthPathways supports greater consistency in referral criteria and improves transparency regarding service access requirements. It also plays an important role in aligning referral information with Clinical Prioritisation Criteria to support safe, appropriate and timely care.
The CPC Program team works closely with HealthPathways teams across the state, Hospital and Health Services, and other key stakeholders to ensure pathways remain current, accurate and aligned with statewide priorities and referral standards.
Through this partnership, HealthPathways continues to strengthen integration between primary and specialist care, reduce variation, and support high-quality patient journeys across Queensland.
