Update the Register

The Register relies on healthcare providers to send patient treatment, management and movement updates to maintain its currency and share up to date information with The Viewer.

Updates can be made to the Register by either:

  • contacting the Register by email, telephone 1300 135 854 or fax 1300 429 536
  • completing and returning the monthly Benzathine Benzylpenicillin G (BPG) recall list
  • completing and returning the quarterly echocardiogram recall list.
Information required to update patient records on the Register
Secondary prophylaxis
  • Patient name and date of birth
  • Date administered
  • Service provider
  • Additional notes (if required) e.g. refusal of or ceased BPG or change to secondary prophylaxis medication
Echocardiogram
  • Patient name and date of birth
  • Date echocardiogram performed
  • Service provider
  • Additional notes (if required)
Specialist review
  • Patient name and date of birth
  • Date of specialist review
  • Service provider
  • RHD Stage (Stage A, Stage B, Stage C or Stage D)
  • Frequency of specialist review (recall frequency)
  • Any changes to prescribed medication or duration of secondary prophylaxis, including ceasing secondary prophylaxis (if required)
  • Planned or completed surgery related to RHD (if required)
  • Current pregnancy (if required)
Movement in and out of community and change of healthcare provider
  • Patient name and date of birth
  • New temporary or permanent location (if known)
  • New temporary or permanent service provider (if known)
