About the Register
The Queensland Rheumatic Heart Disease Register (Register) is a secure statewide disease database that supports the long-term management and care of people diagnosed with acute rheumatic fever (ARF) and rheumatic heart disease (RHD).
Collection of your personal information
Queensland Health collects limited personal information about healthcare providers for the purposes of ensuring appropriate patient follow-up and coordination of care for individuals living with ARF or RHD.
The personal information collected may include:
- your name
- clinic or practice name
- work address
- telephone number
- fax number
- email address.
This information may be colleceted from a range of sources including:
- hospital discharge summaries and clinical correspondence
- details from the initial ARF or RHD diagnosis, recorded on the approved form under the Public Health Act 2005
- referrals for specialist review
- Medicare statements
- Queensland Health’s internal address book directory
- direct contact with health clinics (e.g. confirmation of patient attendance).
Purpose of collection
Your information is collected to:
- Identify and contact the appropriate healthcare providers responsible for the clinical care of patients with ARF or RHD.
- Support continuity of care, patient recalls and follow-up for secondary prophylaxis and specialist management.
- Maintain accurate records in the Register to support integrated care planning and coordination across health services.
Legal authority for collection
Your personal information is collected in accordance with the Information Privacy Act 2009 (Qld). Collection is necessary support public health surveillance and to facilitate patient care under the responsibilities of Queensland Health.
Usual disclosures
Your personal information may be shared with:
- Queensland Health staff and services directly involved in the care of patients recorded in the Register.
- Health care providers involved in the care of patients enrolled on the Register.
- The Register staff, who are responsible for administering the Register, coordinating clinical review, and managing recall coordination and patient follow-up.
Your personal information will be securely stored and managed in accordance with Queensland Health's privacy obligations.
