Request a change to the LAM

Any Queensland Health staff member can request a change to the list of approved medicines (LAM). It is expected that applications for changes will include input from a senior prescriber.

Read the guide for requesting changes to the LAM (PDF 97 kB) before completing the standard submission form (DOCX 240 kB). For minor submissions contact the QHMAC Secretariat.

Requests from pharmaceutical manufacturers or their agents will not be accepted.
