There are 2 ways to access patient resources for advance care planning:

• Order from WINC

Order from WINC

Queensland Health staff will need to be logged into their WINC account to order.

For service providers outside of Queensland Health, please use the following steps:

1. Visit WINC account and log in using these details:
   • Email Address – 1og-statewide
   • Password – Statewide (Capital S is a must when entering Password)
2. Shop by Browse Categories > Printing and the kind of item you are after, or search by product code or SW number (see below).
3. Enter quantity, add to cart
4. Click on cart icon to view cart, continue to Checkout
5. Enter Contact Information – Name, Email and Phone Number
6. Click on “One Time Address” and enter your business address OR click on “Enter Address in Manually”. Enter any delivery instructions if needed
7. Continue to Payment. Enter credit card details – select Pay Now
8. Order completed. Confirmation will be emailed to your email address.

WINC order codes

• SW1174 or SW1175:  SoC forms, including tip sheets (50 pack)
• SW1219: ACP Folder and Resources, including ACP postcard, brochure, SoC (Form A & B) & OPG info sheets (25 pack)
• SW1186 ACP brochures (50 pack)
• SW1262 Advance Care Yarning brochures (50 pack)

The cost includes delivery to anywhere in Queensland. Orders should arrive within 1 business day.
