Apply for an ART licence

Eligibility

To apply for a licence, an ART provider must:​

  • hold current prescribed accreditation​
  • not be banned from providing ART services​
  • meet any extra requirements set by the law.

Information required​

  • Evidence of compliance with relevant industry standards​.
  • Addresses of premises where ART services will be provided, e.g. consultation rooms​.
  • Types of services offered at each premises​.
  • Details of medical practitioners performing, or supervising, ART services ​.
  • Details of personnel engaged in the provision of ART services​.
  • Confirmation of financial sustainability of systems to store records and sperm, eggs or embryos.

The current dollar value of a fee unit is $1.096 as of 1 March 2026.

Initial licence application

Apply for an initial ART licence through the ARTS Portal using the approved online form ‘Licence Application Form – Online’ (ARTU-FM001.v1).

If the ARTS Portal is unavailable, an approved paper application form ‘Licence Application Form – Offline’ (ARTU-FM002.v1) is available upon request by emailing ART@health.qld.gov.au.
