Certified copies of qualifications and proof of identity documents

If you are applying for a licence or certificate in your name (i.e not in the name of a corporation or other entity), and you do not currently hold a licence or certificate issued under the Radiation Safety Act 1999, your application will need to include certified copies of your:

  • qualifications
  • training certificates
  • proof of identity documents.

A certified copy is a copy (often a photocopy) of an original document that is certified or endorsed as a true and complete copy of the original. It does not certify that the original document is genuine.

Those who can certify your proof of identify documents as true and complete copies are detailed on the application form and include:

  • justices of the peace
  • notaries public
  • commissioners for declarations
  • police officers (certifying officer to include name, rank and name of their police station)
  • teachers (certifying individual to include name and teacher registration number)
  • Ahpra registered health practitioners (certifying individual to include name and professional registration number).

Those applying for criminal history and security checks, as related to security enhanced sources, can only have documents certified as true and complete copies from:

  • justices of the peace
  • notaries public
  • commissioners for declarations.

Proof of identity documents

Your application will need to include 1 certified copy of a document from the primary identity document list, and 1 certified copy of a document from the secondary identity document list.

At least 1 of the documents needs to contain a photograph of the applicant.

Primary identity documents

Provide 1 of the following:

  • Australian birth certificate
  • birth certificate from another country accompanied by a passport or Australian visa document issued by the Commonwealth department in which the Migration Act 1958 (Cwth) is administered
  • a travel-related document under the Australian Passports Act 2005 (Cwth)
  • current Australian passport (or not expired for more than 2 years)
  • current foreign passport
  • document evidencing Australian citizenship issued by the Commonwealth department in which the Migration Act 1958 (Cwth) is administered
  • current Australian driver licence (or not expired for more than 2 years)

Secondary identity documents

Provide 1 of the following:

  • current identification card issued by the Commonwealth or State as evidence of your entitlement to a financial benefit
  • account statement issued by a financial institution within the previous year
  • document evidencing discharge from military service within the previous 2 years
  • current student identification card containing your photograph and signature (or not expired for more than 2 years)
  • document evidencing enrolment in a school, university, training institution or professional college within the previous 2 years
  • document evidencing electoral enrolment within the previous 2 years
  • utilities account statement issued by a utilities provider within the previous year
  • notice of land valuation, water rates or council rates issued within the previous year
