The Community Palliative Care Clinic Box contains tailored resources for families, health professionals and clinical services to support the provision of quality end-of-life care for people who choose to be cared for at home.
It is very important to note that the clinical processes for carers managing subcutaneous medicines at home have not changed have not changed and remain consensus-based best practice. Existing resources from individual caring@home packages for carers can still be used.
This free box provides practical information that carers can refer to at any time and is suitable for use in all Australian jurisdictions.
PallConsult has partnered with caring@home to produce a limited number of Community Palliative Care Clinic Boxes to support best-practice palliative care in Queensland.
Queensland services providing community palliative care can order one box per site. The box is designed to be stored centrally and contains resources that can be used with multiple patients and families. These resources can also be downloaded and reprinted as needed to restock the box.
