Documents for SSAs

• HREA or HREC approved research protocol—refer to applying for ethics approval.
• HREC approval letter—refer to submitting an ethics application.
• Study budget and funding approval—a budget must be provided for all projects submitted for governance review.
  • You also have to submit a quote from Pathology Queensland if our research services are required for the project. Get a quote by completing the Application for services form on engaging Pathology Queensland.
  • If your research project is funded by a grant, please upload evidence of funding approval with the SSA application.
• Research agreement—the Pathology Queensland Research Office can assist with the negotiation of research agreements, including obtaining Pathology Queensland legal review where required.
• Quote for pathology services—a quote for services will be provided when an application for services form (DOC, 218.3 KB) is submitted to Pathology Queensland Client Services.
• Laboratory declaration—a laboratory declaration will be provided when an application for services form (DOC, 218.3 KB) is submitted to Pathology Queensland Client Services.
• Public Health Act (PHA) approval—for use of confidential information or samples for research. Researchers wanting to access identifiable or potentially re-identifiable data or samples from Pathology Queensland where consent cannot be obtained from the patient can apply for the release of confidential information under s280 of the Public Health Act (PHA) 2005. • State coroner approval for use of coronial material—Research involving access to coronial material must be referred to Queensland Health's Forensic and Scientific Services Human Ethics Committee (FSS-HEC) for ethical and legal approvals. Written agreements

Non-Queensland Health researchers—Research by non-Queensland Health researchers involving Queensland Health or Pathology Queensland employees, premises, resources or patients and at least one other institution requires a written agreement in accordance with the Australian Code for the Responsible Conduct of Research.

Projects involving two or more organisations which are part of the Brisbane Diamantina Health Partners (BDHP) can use the BDHP Research Passport Agreement. The agreement aims to streamline the approval process for researchers, saving them time and legal costs.

Queensland Health researchers—A research agreement is not required for Queensland Health researchers conducting research with other Queensland Health employees.

Following the submission of a PQ Research Application or Application for Services form and registration of your project, a link to the Pathology Queensland site-specific assessment (SSA) application will be emailed to you.
