Description 
Case Study research explaining the transition for young people with ABI moving from paediatric to adult rehabilitation services

Rationale
Meeting the specialised needs of young people with ABI in transition from paediatric to adult services is difficult to achieve in the current fragmented service system

These inappropriate service systems result in young people opting out of health services, leading to personal crises

Greater understanding of the experience is needed to develop appropriate specialised services for young people in transition is needed to provide continuity of care during this critical period

Young people in transition, their families and paediatric and adult ABI rehabilitation services

Identifying the perceived needs of young people in transition and their families through case studies

Developing appropriate resources to support the transition of young people and their families into adult services
