List of Radiation Health application forms
You may apply for a licence, approval or certificate by one of the following methods:
- If applying for a use licence, lodge an application via our online portal, Public Health Connect (PH Connect) – just follow the prompts to log in to your existing portal account or register for a new account.
- For all other applications, use one of the application forms listed below (noting that you can still apply for use licences via this route if necessary).
