Can I apply?

MASS administers some specialty programs and schemes which may vary in eligibility requirements, please refer to guidelines.

  1. You will need to arrange a clinical assessment by a health professional (clinician) approved by MASS to prescribe the particular type of aid or equipment.
  2. The prescriber will assist you with the completion of application forms and/or submit the application on your behalf.

You may be eligible for assistance under MASS if:

  1. You are a permanent resident of Queensland.
  2. You have a permanent or stable condition or disability.
  3. You hold one of the following eligibility cards in your own name:
    • Centrelink Pensioner Concession Card
    • Centrelink Health Care Card
    • Department of Veterans' Affairs (DVA) Pensioner Concession Card (conditions apply)
    • Queensland Government Seniors Card
  4. You meet the clinical criteria for each aid.

You may be eligible for assistance under Cystic Fibrosis Program (CFP) if:

  1. You are a permanent resident of Queensland.
  2. You hold one of the following eligibility cards in your own name:
    • Centrelink Pensioner Concession Card
    • Centrelink Health Care Card
    • Department of Veterans' Affairs (DVA) Pensioner Concession Card (conditions apply)
    • Queensland Government Seniors Card
  3. You meet the clinical criteria.

CFP Aids and equipment are not provided by the program for hospital inpatients and high
care residents of Commonwealth funded care facilities.

You may be eligible for assistance under Queensland Heat Moisture Exchangers (HME) Subsidy Scheme for Laryngectomy (QHSSL) if:

  1. You are a permanent resident of Queensland.
  2. You hold a current Medicare Card OR an appropriate visa
  3. You have undergone laryngectomy
  4. You have completed an appropriate trial of laryngectomy respiratory consumables for a minimum of four (4) weeks OR providing evidence you are self-funding laryngectomy respiratory consumables.

Palliative Care Programs:

You may be eligible for assistance under PCEP/PCSDP if:

  1. You are a permanent resident of Queensland with a Queensland delivery address
  2. Provide a Medicare Card number for purpose of unique identification. If you do not have one, please contact MASS.
  3. Have a Palliative Confirmation form completed
  4. Meet the clinical criteria for each aid.

You may be eligible under Queensland Artificial Limb Service (QALS) for definitive prosthetic services if:

  • Be aged 65 years or older;
  • be an Australian Citizen, Permanent Resident or a holder of a protected special category visa (SCV);
  • be listed on a current and open resident (Green) Medicare Card
  • have completed an ‘interim rehabilitation program’ at an amputee clinic, been assessed and deemed competent and suitable to use a prosthetic limb.
  • Not be obtaining prosthetic funding support or services through another Government agency or state service.

You may be eligible for assistance under the Spectacle Supply Scheme (SSS) if:

  1. You are a permanent resident of Queensland.
  2. You hold one of the following eligibility cards for a minimum of six (6) months in your own name:
    • Centrelink Pensioner Concession Card
    • Centrelink Health Care Card
    • Department of Veterans' Affairs (DVA) Pensioner Concession Card (conditions apply)
    • Queensland Government Seniors Card
  3. You mhave a clinical need for spectacles as determined by an
    Optometrist/Ophthalmologist.
